Smile Club Membership Plan

Smile Club Membership Plan (SCMP) is a dental savings plan. It is not dental insurance and is not a qualified health plan under the Affordable Care Act. The membership plan is a direct agreement between plan members and their dentist, eliminating the cost and hassle of a middleman.

 

The enrollment fee is a one-time fee due at the start of enrollment. Monthly fees are will be billed once monthly, or can be paid for a year in advance. In the event that a member unenrolls from SCMP, they must enroll again and pay another enrollment fee.

 

Your membership will auto-renew and the credit card or bank account on file will be billed on the anniversary date of your registration each month. There will be no refunds. Rates are subject to change on an annual basis. This plan cannot be assigned, sold, or transferred by the patient. This plan cannot be utilized with any dental insurance or other 3rd party payers.

 

The discount applies to work within the scope of your SCMP provider and is at their sole discretion. This plan and its discounts cannot be combined with any other promotional or discounted services. This discount applies to dental services only and excludes dental products purchased in our office.

 

Discounted payments are due at the time of service. Services not paid in full on the day of treatment are ineligible for a discount and will be billed at our regular rate. Membership participation is required through the completion of any extended treatment plans to receive SCMP discounts. Discounts do not apply to completed services or treatments that are already in progress at the time of enrollment. Discounts can not be applied if payment is made using a 3rd party payer such as CareCredit.

 

We reserve the right to terminate membership at our discretion for nonpayment of membership or dental fees, missed appointments, and/or lack of a healthy doctor-patient relationship. If we do so, we will give you a prorated refund. There is no guarantee of scheduling priority and plan utilization if appointments are canceled.

 

We reserve the right to refuse enrollment to anyone for any reason not prohibited by law. We respect your privacy. Information collected will not be sold to third parties or utilized for any outside marketing purposes.

 

SCMP benefits can only be redeemed at your enrollment location [Smile Clinic, 718 W Coliseum Way, Midvale, Utah]. Terms and conditions of the SCMP are subject to change without notice at any time.

 

Inclusive Benefit Services with annual cost of all plans:

• Basic Dental Cleanings: A total of two cleanings per contract period (12 months beginning at your enrollment date).

• Fluoride: A total of two times per contract period redeemable at the same time as your basic dental cleanings.

• Regular Examinations: A total of two times per contract period redeemable at the same time as your basic dental cleanings.

• Emergency Exam: A total of one time per contract period redeemable at your discretion with no guarantee of scheduling priority.

• Radiographs: A total of two times per contract period redeemable at the same time as your basic dental cleanings.

• Teeth Whitening Kit: One kit per contract period (12 months beginning at your enrollment date).

• General & Cosmetic Dentistry: 10% discount. This applies to basic cleanings, periodontal maintenance, deep cleanings (SRP), fillings, veneers, crowns, bridges, root canals, fluoride treatment, extractions, botox, exams, and radiographs.

• Other treatments may also be discounted at the provider’s discretion.

• Orthodontics: 10% discount. This includes traditional braces and custom clear aligners. 5% discount for Invisalign treatment.

• Implant Dentistry: 10% discount. This applies to implant treatment but excludes full arch treatment such as dentures, overdentures, All-on-4, 3 on 6™, 4 on 8™, etc.

• *Periodontal patients are typically on a schedule of four cleanings per year. The two cleanings that are not included in the Smile Club Membership will be discounted 10%. Periodontal patients that do not follow their cleaning schedule may incur a $150 debridement fee or be charged the full deep cleaning fee.
